Pierced primer ?

I was shooting my 9 mm by Taurus, it's a pt 111 g2.
I have shot it many times before and looked at all the primers afterwards ,but the other day , shooting the Hornady xtp +p 124gr in 9 mm , I had one pierced primer.

I have shot win. 115 gr fmj and Hornady 115 gr critical duty with no problems.

Taurus says +p ammo can be used but not +p+
