What would cause this?

I'm going with defective brass or some sort of debris in the chamber that cut the brass when it was chambered, enough that the ignition and resultant pressure blew it out right there. There's almost no way that's a headspace issue, because it's longitudinal, not perpendicular to the axis of the case. I'd be checking that gun and those loads over very carefully.
 
My guess is that the brass used to make those cases had laminations in the metal. From my experience with deep draw metals defective metal and in particular laminations are all too common.
