Scope, rings, bases....eliminate those as the culprit. I would also try setting aside the piece of brass every time you get a flier. Could be bad runout on that piece of brass or some other issue with it. Is the action bedded, barrel floated?

Like others have said 68gr for 3175fps is right about where I would expect a 7mm rem mag to be. 3150fps with a 150gr and 3250fps with a 140gr. Noslers load data showing 3250fps with a 150 seems on the hot side but rifles vary.

JB bore paste/kroil is the fastest and most painless way that I've cleaned a barrel. I suppose that is subjective but running patch after patch after patch of solvent and never really getting a clean patch drove me nuts before I tried JB/Kroil.
The look of those primers isn't too concerning to me but, you cracked a case head!? Obviously, you shouldn't be jamming bullets into the lands, your barrel could have a short throat and it can easily be lengthened by a gunsmith. If you aren't seeing the results you want, try a different bullet...
